Exactly as the title says:
Dinput/0/ngc usb gamepad is what all remotes register as. Windows recognizes them separately and has different GUIDs on them.
So I have in my device list:
Dinput/0/Keyboard Mouse
Dinput/0/NGC USB Gamepad
Dinput/0/NGC USB Gamepad
Dinput/0/Generic USB Joystick

The NGC USB Gamepad are these http://www.hyperkin.com/pc-mac/pc-mac-premium-gamecube-style-usb-controller-black-cirka.html
The Generic USB Joystick is this: http://www.hyperkin.com/n64-usb-controller-black-cirka.html

While this allows me to play and configure... both of the NGC USB Gamepads only reference 1 of the physical units. So the game effectively doesn't see the other pad even though it sees it.

Things I've already tried:
- Different ports
- Rebooting
- Safe Mode Uninstalling the Hardware
- Different computer
- Different versions of Dolpin (4.0.9103 and latest available)
- Banging the keyboard
- Kicking the neighbor's cat


Any suggestions / help would be greatly appreciated.

Just to be sure, what is the "latest"? 5.0 or the latest development build?

Try the latest development build. If the devices really have the same name, there's a change in the latest builds that should fix the bug.
