Dolphin does not interface directly with HD3D (it does not interface directly with 3D vision either).
What this patch does is to change the way fullscreen mode works so that the Nvidia 3d Vision display driver can use it.
Once this is done, the 3D Vision rendering driver hijacks the DirectX rendering pipeline, records everything the Dolphin DirectX graphics plugin calls and converts the rendering in stereo3D.
On the AMD side, the driver that records the graphics calls and converts them in stereo3D (third party drivers iZ3D and DDD) is separate from the display driver (AMD HD3D).
The iZ3D and DDD drivers do not need the fullscreen mode to work but HD3D does need it.
